Adenosine Injection: Rapid-Acting Antiarrhythmic for Supraventricular Tachycardia (SVT)

Adenosine Injection is a short-acting antiarrhythmic agent primarily used to restore normal sinus rhythm in patients with paroxysmal supraventricular tachycardia (PSVT), including that associated with accessory bypass tracts (e.g., Wolff-Parkinson-White syndrome). It works by slowing conduction through the atrioventricular (AV) node, momentarily interrupting abnormal electrical circuits in the heart and allowing normal rhythm to resume.

  • Adenosine binds to A1 receptors in the heart, reducing calcium influx and increasing potassium efflux, which results in transient AV nodal block

  • Its ultra-short half-life (~10 seconds) allows precise control and rapid therapeutic action

  • Additionally studied for its role in ocular blood flow regulation, with endogenous adenosine being linked to glaucoma pathophysiology

  • Therapeutic Uses

    • Paroxysmal Supraventricular Tachycardia (PSVT)

    • Wolff-Parkinson-White Syndrome (as a diagnostic and therapeutic agent)

    • Pharmacologic stress testing (in myocardial perfusion imaging, off-label)

    • Investigational role in glaucoma therapy (via modulation of intraocular pressure)
